We foster a culture where every team member matters, and our leadership team is actively involved and supportive, ensuring staff feel heard, valued, and empowered.Why Join Us?Competitive salary: $80K–$85KMonday–Friday scheduleUnlimited employee referral bonusesFun employee appreciation events & rewardsMedical, dental, vision & life insurance optionsAdvancement opportunities across South CarolinaPRN opportunities within our networkPosition SummaryWe are seeking an experienced MDS Coordinator (RN) to ensuring accurate and timely completion of MDS assessments in compliance with state, federal, and Medicare guidelines.Key ResponsibilitiesComplete and submit MDS assessments accurately and on timeEnsure compliance with CMS, Medicare, and Medicaid regulationsCoordinate with the interdisciplinary team for care plan developmentReview and monitor documentation to support reimbursement accuracy (PDPM)Participate in care plan meetings and QA processesTrack and manage assessment schedules and deadlinesEducate staff on documentation and regulatory requirementsCollaborate with nursing, therapy, and administration teamsQualificationsActive RN license in South Carolina (in good standing)Prior MDS experience requiredStrong knowledge of PDPM, Medicare/Medicaid, and long-term care regulationsExperience in skilled nursing facility (SNF) setting preferredExcellent attention to detail and organizational skillsStrong communication and teamwork abilitiesWhat Makes You a Great Fit Detail-oriented with strong analytical skills Organized and deadline-driven Collaborative and team-focused Passionate about quality care and complianceWe are an equal opportunity employer.
