What You’ll Do Perform in-home assessments to address activities of daily living (ADLs), instrumental ADLs, and home safety. Develop adaptive strategies: create personalized treatment plans focusing on energy conservation, use of adaptive equipment, and functional strengthening for daily tasks. Manage your visits: schedule and conduct patient home visits based on your preferred caseload and availability. Collaborate for coordinated care: work with a supportive team to ensure a holistic approach to each patient’s plan of care.
