Compassionate Hospice RN Case Manager

Snowline Health in Rancho Cordova, CA, is seeking a dedicated Registered Nurse Case Manager to join our hospice care team. You will provide compassionate nursing care, educate families, coordinate care, and advocate for patients. The role requires a California RN license, BSN/ASN/diploma, at least one year of nursing experience (hospice preferred), strong communication, and ability to travel weekly within El Dorado County.
