JOB SUMMARY
The Case Manager monitors, completes assessments, and develops service plans for wards of the State in Cook County who are six years of age and older. The position maintains comprehensive case records for at-risk clients and provides medical case management services, including referrals, linkages, and follow-up with providers, IDOCFS case workers, foster parents, and guardians.
The Case Manager conducts outreach to clients as needed, including home visits, to identify, address, and reduce barriers to accessing services. The position ensures clients receive needed and required services and maintains ongoing communication with assigned clients and their support systems.
JOB DUTIES
Provide direct medical case management services to assigned clients.
Maintain ongoing contact with assigned clients.
Complete client assessments and follow-up activities for assigned caseload.
Develop and maintain individualized service plans for assigned clients.
Monitor clients' progress and ensure identified needs and service requirements are addressed.
Provide referrals and link clients to appropriate medical, behavioral health, community, and supportive services.
Conduct outreach and home visits as necessary to increase access to services and reduce barriers.
Work collaboratively with providers, IDOCFS case workers, foster parents, guardians, lead agencies, and other members of the client's support system.
Ensure clients receive appropriate and coordinated services based on identified needs.
Maintain comprehensive, accurate, and timely client records.
Complete required documentation and data entry for assigned caseload.
Prepare reports and case information as required.
Participate in case reviews and other meetings related to assigned clients.
Advocate on behalf of clients to assist with accessing appropriate services and resources.
Participate in professional development and training opportunities.
Maintain effective communication and working relationships with management, administrative staff, clients, families, and community partners.
Perform other related duties as assigned.
QUALIFICATIONS
Bachelor's degree, preferably in Behavioral Science, Social Science, Sociology, Psychology, or a related field.
Experience in case management and/or working with at-risk populations.
Experience working with children, families, and/or community-based populations preferred.
Knowledge of problem-solving skills and the ability to collaborate effectively with clients, families, providers, and community partners.
Strong communication and interpersonal skills.
Ability to analyze and solve problems.
Strong organizational skills with the ability to plan and manage multiple clients and activities.
Ability to develop and maintain effective working relationships with management and administrative staff.
Ability to gather data, compile information, and prepare reports.
Proficiency with computerized systems and databases.
Must have effective time-management skills.
Must have access to a vehicle and possess a valid driver's license.

WORKING CONDITIONS AND PHYSICAL REQUIREMENTS
Ability to move freely and access a variety of locations.
Moderate physical activity may be required.
Work is generally performed in an office/interior environment as well as community-based settings.
Position may require travel to client homes, provider locations, and other community sites.
Must be able to safely perform the essential functions of the position with or without reasonable accommodation.

Occupational Risk for Exposure to TB:
There is an occupational risk for exposure to TB, as this position involves extensive community and patient contact. Employees in this role are required to comply with annual TB testing as part of workplace health and safety protocols. The only acceptable exceptions to this requirement are documented medical contraindications or religious objections. Employees who decline TB testing will be required to wear a mask when deemed necessary.
