This flexible position allows you to tailor your own schedule, serving members throughout Luna County, including Deming, Columbus, and City of the Sun. Home health visits include annual risk assessments and post-discharge assessments, each appointment averaging about 45 minutes. Your role will help ensure comprehensive care planning and health management for older adults in their own homes.Schedule:Flexible shifts, Monday-SundayCreate your own scheduleMinimum weekly or bi-weekly hourly commitmentKey Responsibilities:Conduct comprehensive home health risk assessments for Medicare Advantage membersEvaluate current/prior medical history, active conditions, and medication listsEducate members on health conditions and medication regimensIdentify gaps in care and provide actionable recommendationsNo requirement to prescribe medications or create treatment plansQualifications:Active Nurse Practitioner license in New MexicoBoard Certification through AANP or ANCCCurrent CPR certification (BLS)Masters degree in Nursing (MSN) from an accredited programMinimum of 6 months NP experience preferredProviders will receive competitive pay on a per-evaluation basis, inclusive of a mileage stipend and administrative time.
