Mental Health Care Manager (HH+) - Care Coordinator

Argus Community, Inc. is seeking a Mental Health Care Manager (HH+ SMI) in the Bronx to coordinate care for up to 20 clients under NYSDOH guidelines. You will assess needs, develop client-centered care plans, and collaborate with providers and MCOs. Responsibilities include home visits, regular contact with clients, case conferences, crisis intervention, and accurate documentation to support Health Home functions.
