Optum
Optum is seeking a Home Health Patient Care Manager in the Dallas area to supervise and coordinate clinical services for home health patients. You will lead an interdisciplinary team to ensure high-quality care in accordance with physician plans and state/federal regulations. Responsibilities include receiving referrals, starting care, coordinating benefits and approvals, and daily review of plans of care.
