Medical Care Coordinator

Location: Hicksville, NY | Schedule: FT 35 hours a week | Status: Non-Exempt | Salary Range: $24-28/hour


The Opportunity

The Medical Care Coordinator supports clients and the clinical team by coordinating prescription refills, prior authorizations, medical paperwork, managing medication supply, and financial assistance applications. The position communicates with clients, providers, and pharmacies, documents medication concerns and potential side effects, processes disability and benefits paperwork, maintains accurate health records, and protects client confidentiality while supporting timely and continuous care at FCA's Certified Community Behavioral Health Center (CCBHC).

Responsibilities
• Coordinate prior authorizations for clinical, psychiatric, and prescription medications by gathering medical documentation, submitting requests to insurance companies and PBMs, and tracking approvals to prevent interruptions in treatment.
• Manage prescription refill requests, communicate with pharmacies and providers, route requests according to established protocols, and provide clients with timely status updates.
• Receive and document client concerns regarding medication symptoms or potential adverse reactions and promptly communicate clinically relevant information to the treatment team.
• Process disability eligibility forms, medical documentation requests, and other paperwork related to healthcare and other benefits.
• Review financial assistance applications for fee adjustments by collecting and verifying financial documentation, including pay stubs, tax records, and household-size information, and determining eligibility based on Federal Poverty Guidelines.
• Collaborate with care coordinators, clinicians, pharmacies, insurance representatives, and clients to identify and resolve barriers to medications, appointments, and behavioral health services.
• Accurately document prior authorizations, refill communications, manage medication supply, financial eligibility, patient interactions, and other clinical and administrative activities within the Electronic Health Record (EHR).
• Maintain strict confidentiality and compliance with HIPAA requirements while supporting CCBHC quality standards and continuity of care.

Qualifications
• Bachelor's degree in human service and health fields
• Minimum of 1-2 years of experience working as a medical assistant or healthcare administrator; graduation from an accredited Medical Assistant program or equivalent administrative medical billing/front-office experience preferred.
• Experience managing prior authorization submissions and electronic prescription platforms.
• Knowledgeable with medications prescribed for mental health and substance use
