Whole Person Care Clinic

The Whole Person Care Clinic is seeking a Case Manager to provide direct, trauma-informed case management to individuals experiencing homelessness and Recuperative Care participants. You will bridge acute medical care, behavioral health, housing stability, and community support services with compassion and advocacy. Responsibilities include intake assessments, developing service plans, coordinating care across providers, and ensuring accurate documentation in HMIS to support seamless discharge #J-18808-Ljbffr Whole Person Care Clinic
