Position Summary: The Music Therapist (MT-BC) works as a member of the interdisciplinary team working with terminally ill patients and their families. The MT-BC will provide music therapy services for assigned hospice patients and families, in accordance with the interdisciplinary plan of care. During an initial evaluation, MT-BC will assess the patient’s strengths and needs. MT-BC will develop and implement a care plan, evaluate outcomes and perform required paperwork and team communication in a timely manner. MT-BC will manage their own schedule for making patient visits; taking into consideration: changing patient needs, team needs and meetings. Team: Respectfully working together to achieve the best end of life experience for our patients and families; promoting a positive work experience Qualifications: Bachelor’s degree or equivalent in Music Therapy from an accredited music therapy program Current board certification from the Certification Board for Music Therapists (CBMT) Minimum of 1-year experience in music therapy; 2+ years’ experience preferred Experience in Hospice preferred Must be a licensed driver with auto insurance and reliable daily transportation Able to work independently and as a member of the interdisciplinary team Duties: Attend Interdisciplinary Group (IDG) meetings, music therapy team meetings and all staff meetings Collaborates with the interdisciplinary team to provide support to patient and families Documents patient visits to reflect individualized care provided to patients in a timely manner Provide music for staff support and special Grace events as needed Provide educational in-services to the community and other professionals. Utilize GarageBand computer software to create legacy gifts for patient families.
