Duties
Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
Monitor and observe patient health status, including vital signs and overall well-being, while accurately documenting medical information in Electronic Health Record (EHR) systems
Administer medications following medical instructions and ensure proper medication documentation in compliance with home healthcare regulations and HIPAA privacy standards
Provide specialized care for patients with dementia, Alzheimers disease, or other cognitive impairments, including memory care strategies and behavior management
Support patients with complex needs such as catheter care, hospice care, end-of-life assistance, and managing medical devices or equipment
Prepare nutritious meals tailored to dietary restrictions and assist with feeding as needed; perform light housekeeping tasks like cleaning and laundry to maintain a safe environment
Use medical terminology confidently to communicate effectively with healthcare teams and document patient progress accurately
