Inpatient Care Manager RN Acute Care & Discharge Leader
University of Minnesota School of Nursing
Atlantic Health is seeking a Nursing Case Manager to oversee care across the illness continuum, coordinating assessments and multidisciplinary plans to maximize patient outcomes. You will monitor chart reviews and utilization, lead rounds, and drive efficient discharge planning while working with an interdisciplinary team to optimize pathways.
The role requires a Nursing degree and NJ licensure, with preferred Acute Care experience.
