Min $46.78 Max $79.58 Flex Schedule Everythother weekendDefinition of PositionAssess, develop, implement and monitor a comprehensive plan of care through an interdisciplinary team process in conjunction with the patient and family in internal and external settings. Comprehensively assess patients' goals as well as their biophysical, psychosocial, environmental, functional, and financial status. Procures services and serves as advocate on behalf of patients and families for scarce resources. Directs families to arrange for services which require patient/family decision and participation. Acts as liaison to post-hospital care providers and community health resources. Communicates continually with patients, families, medical and hospital staff, caregivers, and post hospital resources. Facilitates or provides necessary patient and family education prior to admission, during hospital stay, and after discharge as indicated. Relates well to children, adolescents, adult and geriatric clients.QualificationsEducation: Required: Graduate of an accredited school of medicine, social work, nursing or related medical occupation. Preferred: Previous experience with coordinating community resources for post-acute hospitalization care. BSN/MSN. Licenses/ certifications/ registrations: Required: Active Oregon RN License Preferred: Case Management Certification Experience: Required: Previous clinical experience in an acute care setting, or home health setting assessing skilled care needs and equipment and health maintenance resource needs. Preferred: Previous Discharge Planning and/or Case Management experience. Other: Bilingual and/or English Spanish speaking preferred. The position also involves occasional stooping, kneeling, crouching, or crawling when assisting patients or accessing equipment. Walking and standing are typically continuous throughout the shift. The role requires the ability to lift to 50 pounds independently and involves frequent pushing, pulling, carrying, or repositioning of objects or patients weighing up to 25 pounds. There may also be occasional lifting or transferring of individuals weighing over 100 pounds, with the expectation that appropriate lifting techniques, mechanical aids, or team assistance will be used to ensure safety. May sit for long periods of time.Working ConditionsThis position is performed primarily in an indoor healthcare environment, such as a hospital, clinic, or long-term care facility. While the work setting is protected from outdoor weather conditions, it may be subject to variable indoor temperatures. The role involves frequent exposure to infectious and contagious diseases, hazardous materials, and medical equipment, with the expectation that all staff follow appropriate safety protocols and utilize personal protective equipment (PPE) as required. The health care environment can be fast-paced and unpredictable, requiring the ability to respond quickly to changing circumstances, including high-pressure or emergency situations.
