PRP Worker - Rehabilitation Coordinator

Responsibilities: Provide Psychiatric Rehabilitation Program (PRP) services to individuals with mental health diagnoses in community and home settings Assist in developing and implementing individualized rehabilitation plans in collaboration with Rehab Specialist and clients and treatment teams Coach clients on daily living skills, social skills, and community integration strategies Monitor and document client progress toward rehabilitation goals in accordance with program requirements Coordinate with case managers, therapists, and other service providers to ensure continuity of care Transport and accompany clients to community-based activities and appointments as needed Maintain accurate, timely documentation and progress notes in compliance with Maryland COMAR regulations Requirements: High school diploma or GED required; bachelor's degree in psychology, social work, or related field preferred Prior experience working in psychiatric rehabilitation, mental health, or human services Familiarity with Maryland PRP regulations and COMAR standards is a plus Strong interpersonal and communication skills with a client-centered approach Ability to work independently in community and home-based settings Valid driver's license and reliable transportation required CPR/First Aid certification or willingness to obtain upon hire About Us: New Day Health Systems is a trusted behavioral health provider serving the greater Catonsville, MD area, dedicated to empowering individuals on their journey to mental wellness and independence.
