A growing healthcare organization is seeking Nurse Practitioners to partner with dialysis centers and care teams, supporting patients with complex kidney conditions in a value-based care model.

Highlights

MondayFriday schedule no weekends
Work with complex, high-acuity patients (CKD/ESKD)
Collaborative model with dialysis teams and specialists

The Role

Provide primary care support for patients with chronic kidney disease
Manage high-risk patients beyond basic wellness visits
Play a key role in transitions of care and reducing readmissions
Collaborate closely with dialysis and interdisciplinary teams
Build meaningful, ongoing patient relationships

Ideal Candidate

Active California Nurse Practitioner license and Board Certification
Experience in primary care, internal medicine, or complex care settings
Comfortable managing high-risk or medically complex patients
Interest in value-based care and outcomes-driven medicine
Strong communicator who thrives in a team environment
