Coordinate care across different settings and facilitate smooth transitions. Assess and adjust care plans based on evolving patient needs, promoting cost-effective, quality outcomes. Engage in education and innovative approaches to behavioral health in long-term care environments. Qualifications & Skills Unrestricted NP license and PMHNP certification. Active DEA licensure or ability to obtain. Minimum 1+ year experience in long-term or community care settings. Reliable transportation and valid driver’s license for travel between care sites.
