At CareBuilders at Home , we are dedicated to providing high-quality, non-medical home health care services. We pride ourselves on maintaining the highest standards of care, safety, and operational excellence for our clients. We are seeking a driven and experienced  Branch Manager – Home Health to lead and grow our West Palm Beach, Florida market. In this role, you hold full revenue growth and operational accountability for the branch, managing daily recruitment pipelines while building, managing, and expanding a strong network of referral partners across healthcare and community-based channels.

The ideal candidate is a proven relationship builder with established contacts and a track record of generating consistent referral volume in home care, home health, hospice, or related healthcare services.

Responsibilities:

• Develop and execute a territory growth strategy to drive consistent referrals and revenue
• Build and maintain relationships with high-value referral sources, including:
• Skilled Nursing Facilities (SNFs)
• Assisted Living / Independent Living communities
• Hospitals and case management teams
• Physician groups and concierge practices
• Geriatric Care Managers (GCMs)
• Elder law attorneys and fiduciaries

 

• Conduct daily field visits and structured outreach (50–60 touches per week)
• Identify and convert new referral sources while expanding existing relationships
• Position CareBuilders as the preferred provider for private duty home care services
• Coordinate closely with operations to ensure timely starts of care and service excellence
• Track all activity and pipeline in CRM, ensuring visibility and accountability
• Represent CareBuilders professionally in the community, including events and networking opportunities

 

Qualifications:

• 3+ years of healthcare sales or business development experience (home care, home health, hospice, or related field strongly preferred)
• Demonstrated success in building referral networks and driving census growth
• Established relationships with key referral sources in the Fullerton / North Orange County market preferred
• Strong understanding of private pay home care and care transitions
• Proven ability to work independently in a field-based, performance-driven role
