Job Summary:
The position is for a Physical Therapist Outpatient at a healthcare facility located in Beaver Dam, Wisconsin. This outpatient role involves providing physical therapy services within a smoke-free environment. Candidates are required to bring their license on the first day.

Job Details:

• Location: Beaver Dam, Wisconsin
• Schedule: Monday through Friday, 8-hour days from 7:00 am to 6:00 pm, with a 1-hour lunch break.
• Required Qualifications: Valid Physical Therapist (PT) license; CPR certification from the American Heart Association (AHA); and zero years of experience.
• Certifications: BLS (AHA)
• Experience: 0 year(s)
• Additional Information:
  • Unit type: Outpatient
  • EMR: EPIC
  • Shift notes: M-F, 8 hours/day, 7am-6pm, 1-hour lunch
  • Dress code: polos and professional athletic pants (athleisure)
  • All drug reports must have a negative result for nicotine/cotinine; positive results will be reviewed if the candidate is prescribed a nicotine patch. Reports with no legitimate reason for positive results will result in contract cancellation.
  • Cancellation policy: Three shifts per 13-week assignment or two shifts for assignments 12 weeks or less.
