Hospice RN Case Manager - North Brooklyn, NY

Job Summary We are hiring a full‑time Hospice RN Case Manager to join our North Brooklyn hospice team. This field / community‑based RN role supports hospice patients and families in a focused territory, providing meaningful end‑of‑life care while maintaining manageable visit expectations. Territory & Schedule North Brooklyn, NY – field/community based Patient visits in homes and facility settings, clustered by nearby zip codes Monday through Friday, 8:30 AM – 5:00 PM Approximately 4 hospice visits per day; visits usually completed earlier in the day Charting, care planning, and coordination calls often completed from home in the afternoon Benefits $15,000 sign‑on bonus (subject to position) Pension plan fully funded by employer; vesting at 5 years Employer contribution of 5.75% of salary per year to 403(b) retirement plan 4 weeks of vacation; 8 company‑paid holidays Low‑cost medical, dental, and vision coverage for employees and family members Medical coverage through Cigna; dental through Cigna PPO or Aetna DMO; vision through UnitedHealthcare Free online RN to BSN and MSN degree programs; annual tuition reimbursement up to $4,500; dependent tuition reimbursement for clinical staff 529 college savings plan; flexible spending account; dependent care FSA Employer‑paid life insurance at 2x annual salary; short‑term and long‑term disability through MassMutual Employee Assistance Program; commuter benefits for transit and parking; pet care discounts with participating providers Responsibilities Manage an assigned hospice caseload of terminally ill patients Provide skilled nursing care in home and facility‑based settings Support patients and families throughout the end‑of‑life process Develop and update individualized hospice plans of care Complete physical, psychosocial, and symptom‑management assessments Establish patient‑centered goals of care based on patient and family needs Educate patients and families on disease progression, medication administration, and symptom management Collaborate with physicians and the interdisciplinary hospice team Coordinate services outlined in the hospice plan of care Assist with medication titration and clinical care updates as appropriate Assess need for medical equipment, supplies, and safe use in the home Coordinate with office staff and vendors for timely equipment and supply delivery; anticipate supply needs before weekends and holidays Follow infection‑control standards and organizational policies Serve as a clinical resource and liaison for patients in crisis when needed Maintain accurate, timely clinical documentation Qualifications Active New York State RN license in good standing At least 1 year of RN experience required; hospice, palliative care, med/surg, or oncology experience preferred BSN strongly preferred Comfortable working independently in the field Strong communication, organization, and patient/family education skills Passion for hospice, palliative care, and end‑of‑life support Equal Opportunity Employer Jobot is an Equal Opportunity Employer.
