Pride health is actively hiring a Registered Nurse to join our client's home health (Hospice) program in a growing care team serving patients .

Compensation:

• Local Pay: $50.00 to $55.00/hour (W-2)
• Travel Pay: $2,400 to $2,500 gross per week
• Mileage Reimbursement: $0.49/mile (from first patient to last patient)

Location: Fayetteville, NC, 28304

Schedule details (hours per day/week): 40

Productivity expectations: min 25

On Call/Weekend Requirement: Yes about 1-2 weekday and 1-2 weekend ( I currently have a 7on/7off on call nurse so on call is covered by field staff on his off weeks.

Coverage Areas: 50 miles from office

Shift schedule (days, start & end times): Monday to Friday, 08:00 AM to 4:30 PM; On Call starts Monday to Friday 04:30 to 8:00 am and Sat/Sun 8am - 8am

Responsibilities:

• Provide comprehensive hospice case management by coordinating and delivering patient-centered end-of-life care, completing admissions, developing and updating individualized care plans, collaborating with the interdisciplinary team, and maintaining accurate PointCare documentation while meeting productivity expectations.
• Manage a 50-mile patient territory by conducting home visits, monitoring patient conditions, educating patients and families, responding to on-call needs (weekday/weekend rotation), ensuring timely interventions, and maintaining CPR certification and compliance with hospice care standards.
