Description:
As an integral part of a team, the Virtual Care Coordinator is responsible for supporting virtual patient care activities and promoting patient-focused care.
This role is responsible for performing various remote monitoring duties involving patient monitoring for safety and clinical needs.
The coordinator provides support and assistance with telemedicine services and monitors environmental status.
The Virtual Care Coordinator collaborates on implementing planned telehealth and remote patient monitoring initiatives.
This role includes collaboration with leadership to coordinate training and participation in developing telehealth best practices.
Clear communication and collaboration with the clinical team, patients, technology partners, engineering, platform vendors, and other managerial duties are essential.
The coordinator shares accountability for clinical quality and safety outcomes.
Essential Duties and Responsibilities include:
Performing quality assurance and peer reviews.
Evaluating telehealth and virtual patient observation programs in collaboration with leadership and making recommendations for improvement.
Meeting regularly with senior leadership to discuss quality metrics and provide feedback on workflow expansion.
Collaborating and leading case reviews with partners to implement solutions and monitor outcomes.
Triaging and assigning cases for virtual monitoring and observation.
Collaborating on telehealth policy and workflow development and implementation.
Running and maintaining operational databases for program performance and quality metrics.
Identifying concerning trends and participating in developing corrective action plans.
Working with staff to efficiently utilize telehealth technology to achieve system goals.
Successfully completing Virtual Care Coordinator orientation.
Providing training on telehealth workflows, platform applications, and documentation requirements to new staff.
Serving as a liaison between physicians, clinical staff, technology partners, and vendors regarding telehealth issues.
Facilitating camera utilization by analyzing patient care utilization reports and assessing eligibility for virtual care.
Coordinating transition with hospital care teams based on patient criteria reports.
Navigating multiple technology platforms and working with healthcare providers to ensure quality care and outcomes.
Providing real-time support to troubleshoot issues related to telehealth and remote monitoring platforms and equipment.
Conducting continuous observation and surveillance of assigned patients.
Monitoring clinical early warning signs such as vital signs and other metrics.
Using interpersonal skills to effectively communicate and collaborate with patients and the clinical team.
Immediately reporting any variations or concerns in patient status to the appropriate clinical staff.
Documenting patient behavior and redirection interventions.
Suggesting patient discharge from remote monitoring based on observed behavior.
Monitoring scheduled telehealth consult orders and ensuring sessions occur without issues.
Greeting patients, families, and visitors, and introducing self and role.
Promptly answering calls from clinical teams.
Using approved communication methods with clinical teams.
Assisting in interpreting through approved methods, as needed.
Ensuring patient privacy is protected by adhering to information security policies and HIPAA guidelines.
Reporting patient safety concerns and escalations as needed.
Maintaining accurate records of interventions and safety events.
Setting priorities, problem-solving, and exercising judgment in difficult situations are required.
Sufficient oral and written skills in the English language to communicate effectively are essential.
Submitting utilization and assignment reports according to departmental procedures.
Attending staff meetings and regularly checking work email for updates.
Maintaining attendance and punctuality records in accordance with departmental policy.
Maintaining a safe, clean, and functional environment.
