Resident Care Coordinator - Senior Living

Resident Care Coordinator – Senior Living Chubbuck, ID Savoy Life is a distributed senior living platform focused on bridging the gap between senior living communities and healthcare. With a team of specialized clinicians, including nurses and geriatricians, Savoy Life offers a tech-enabled solution that empowers senior living operators to do their jobs more effectively and efficiently, enabling faster community transitions and longer periods of better health for residents. Location: Travel within Chubbuck & Pocatello, ID Work Schedule: Full-time Note: This role is mostly in various senior living settings and may include varying levels of drive time during the day to these senior homes and communities. The job is 8am-5pm plus drive time prior to 8am to the first senior and drive time after 5pm from the last senior’s home. About Savoy Life: Savoy Life is a technology‑enabled healthcare services startup focused on making healthcare work for seniors and their caregivers. The company operates both in individual homes and in partnership with senior living communities to coordinate and manage clinical gaps for seniors, their caregivers, and the care ecosystem. Savoy deploys a team of clinical experts—both in‑person and virtually—paired with proprietary technology to drive clinical outcomes without disrupting current care pathways. Position Overview: Savoy Life seeks a dedicated Resident Care Coordinator (RCC) to provide clinical and operational support to seniors in and around Chubbuck & Pocatello, ID. The RCC will work closely with an assigned Savoy Life RN and a Savoy provider to deliver healthcare in various settings, including assisted living, independent living, memory care units, and individual homes. Key Responsibilities: Care Delivery Back Office Support Schedule patient visits in the EHR to optimize RN and APP time while coordinating with patient and caregiver needs. Answer inbound calls, text messages, and voicemails in a professional and timely manner. Monitor and manage inbound and outbound faxes. Enter data, scan files, and print documents as needed to support patient care and maintain updated historical records in the EHR. Identify and manage health plan insurance for residents. Care Coordination Support Support the RN and Advance Practice Provider (APP) by coordinating patient care to drive outcomes. Collaborate with local Savoy team and community resources to support patient care. Manage facilitated telehealth visits with Savoy’s virtual provider via a tablet. Support ongoing care with direct RCC‑led resident visits. Manage communications and foster positive relationships with patients, caregivers, families, primary care providers, specialists, pharmacies, and senior living community staff. Drive engagement with eligible residents, POAs, and families through education on Savoy’s offering to obtain consent forms. Qualifications: High School Diploma / GED (or higher) 2+ years of experience in healthcare with a focus on care coordination and healthcare administrative support Medical Assistant, Patient Care Technician, or equivalent certification preferred, otherwise 5+ years of healthcare experience Experience in geriatric care or senior living community is highly desirable Must possess a valid and current driver's license & be comfortable driving between senior home settings, senior living communities, and individual homes Excellent electronic medical record (EMR) documentation skills and comfort leveraging various technologies to support day‑to‑day work Ability to juggle many tasks and switch between visiting residents and coordinating behind the scenes Purpose‑driven with a positive attitude to contribute to a strong culture and passion for improving senior living Flexibility to adapt to a dynamic and growing startup organization as the role and company evolve 100% employer paid, comprehensive health care including medical, dental, and vision for you and your family Paid maternity and paternity for 14 weeks at employees’ normal pay Unlimited PTO, with management approval Opportunities for professional development and continued learning Optional 401K, FSA, and equity incentives available Hourly Rate: $20 - $25 We’re an Equal Opportunity Employer: You’ll receive consideration for employment without regard to race, sex, color, religion, sexual orientation, gender identity, national origin, protected veteran status, or on the basis of disability. #J-18808-Ljbffr
