Resident Care Coordinator - Senior Living

$20 - $25 per hour

Position Overview Savoy Life is seeking a dedicated Resident Care Coordinator (RCC) to provide clinical and operations support to seniors in and around Chubbuck & Pocatello, ID. The RCC will work closely with an assigned Savoy Life RN and provider to deliver care across senior living settings (assisted living, independent living, memory care units) and individual homes. Location Travel within Chubbuck & Pocatello, ID Work Schedule Full-time Note This role is mostly in various senior living settings and may include varying levels of drive time during the day to these senior homes and communities. The job is 8am-5pm plus drive time prior to 8am to the first senior and drive time after 5pm from the last senior’s home. Key Responsibilities Care Delivery Back Office Support Schedule patient visits in the EHR to optimize the RN and APP time while coordinating with patient and caregiver needs. Answer inbound calls, text messages, and voicemails in a professional and timely manner. Monitor and manage inbound and outbound faxes. Enter data, scan files, and print files as needed to support patient care and maintain updated historical records in the EHR in an organized and timely manner. Identify and manage health plan insurance for residents. Care Coordination Support Support the RN and APP by coordinating patient care to drive patient outcomes. Work with the local Savoy team to collaborate with community resources that aid in Savoy’s goals of supporting patient care. Manage facilitated telehealth visits with Savoy’s virtual provider via a tablet. Support ongoing care with direct RCC‑led resident visits. Manage communications and foster positive relationships with patients as well as others involved including caregivers, families, primary care providers, specialists, pharmacies, and senior living community staff. Drive engagement with eligible residents, POAs, and families through education on Savoy’s offering to get consent forms signed. Qualifications High School Diploma / GED (or higher). 2+ years of experience in healthcare with a focus on care coordination and healthcare administrative support. Medical Assistant, Patient Care Technician, or equivalent certification preferred; otherwise 5+ years of healthcare experience. Experience in geriatric care or senior living community highly desirable. Valid and current driver’s license and comfortable driving in various senior home settings. Excellent electronic medical record (EMR) documentation skills and general comfort leveraging technologies. Ability to juggle many tasks and switch between visiting with residents and coordinating behind the scenes.
