Care Coordinator

We are seeking a Care Coordinator for the Children's Long Term Support (CLTS) Waiver Program to support children with developmental, physical, or emotional disabilities. Key Responsibilities Include Conducting functional assessments and developing individualized service plans Authorizing and coordinating services aligned with care plans Facilitating family-centered team meetings and maintaining accurate documentation Collaborating with service teams and participating in staff development Promoting client independence through trauma-informed, family-centered practices Providing evidence-based interventions and completing timely documentation Qualifications Required Bachelor's degree in a health or human services field At least 1 year of experience working with children in the target population Strong communication, organizational, and documentation skills Proficiency with electronic health records and computer applications Valid driver's license and reliable transportation Bilingual fluency in Spanish is preferred but not required Additional Details Community-based role with daily travel and occasional overnight stays Work involves responding appropriately in crisis situations Benefits include health insurance, paid time off, retirement plan, and flexible work options #J-18808-Ljbffr Fairview Hospital / Cleveland Clinic
