Responsibilities
- Assess patients and manage hospice symptoms, implementing individualized care plans with the interdisciplinary team
- Deliver comprehensive nursing care to an assigned caseload, including medication administration, vital signs, and symptom control
- Educate patients and families on goals of care, treatment options, and self-care strategies to support informed decision-making
- Collaborate with physicians, social workers, chaplains, and other clinicians; coordinate care across home, nursing home, group home, or assisted living settings
- Document care plans, progress notes, and care conferences; ensure accurate and timely communication within the team
- Maintain compliance with hospice regulations and organization policies; participate in quality and safety initiatives

Requirements
- Active Registered Nurse license (state-specific as required for assignment)
- Ability to practice in a hospice care setting and participate in on-call rotations
- Strong assessment, critical thinking, and interpersonal communication skills
- Demonstrated ability to work with an interdisciplinary team and manage a patient caseload
- Commitment to patient- and family-centered hospice care
- Prior hospice or palliative care experience is preferred but not mandatory
