The Role
We're seeking a dedicated RN Case Manager, specializing in nursing and geriatric care management. Ideal candidates will have at least a year of RN experience and demonstrate:
- Strong teamwork skills, aiding in patient and colleague support.
- Excellent communication abilities with patients, caregivers, and team members.
