Hospice RN Case Manager (RNCM)

Serenity Hospice PA is seeking a compassionate and dedicated Hospice RN Case Manager (RNCM) to join our growing team serving patients throughout Dauphin County, PA.

Responsibilities include:

• Managing and coordinating hospice patient care plans
• Performing comprehensive nursing assessments
• Providing symptom management and patient education
• Collaborating with physicians, caregivers, and interdisciplinary team members
• Maintaining accurate and timely documentation
• Supporting patients and families emotionally throughout the hospice journey

Qualifications include:

• Active Pennsylvania RN license
• Hospice or home health experience preferred
• Strong clinical and organizational skills
• Excellent communication and compassionate patient care abilities
• Valid driver's license and reliable transportation
