Physical Therapist / Multi-Setting Rehabilitation

A healthcare organization in Southeastern Colorado is seeking a full-time Physical Therapist to join its Rehabilitation team.

This is a strong opportunity for a PT who enjoys variety in practice settings, including inpatient, outpatient, swing bed, and home health. New graduates and experienced Physical Therapists are welcome to apply. Job Responsibilities

In this role, you will:

• Evaluate, consult, and treat patients referred for physical therapy services.
• Develop and implement individualized plans of care to help patients return to prior level of function or reach maximum medical improvement.
• Provide care across multiple settings, including inpatient, outpatient, swing bed, and home health.
• Treat a broad patient population, including pediatric, adolescent, adult, and geriatric patients.
• Complete initial assessments, reassessments, daily notes, progress reports, and other required documentation.
• Supervise and support Physical Therapist Assistants as appropriate.
• Provide education and instruction to patients, families, supportive staff, volunteers, and students.
• Communicate effectively with physicians, clinical staff, patients, families, and administration.
• Adapt to a changing clinical environment while maintaining quality, safety, and patient-centered care.

Schedule

• Full-time position.
• Monday-Friday schedule.
• Occasional weekend coverage for inpatient and swing bed evaluations.
• Extended hours may be needed based on department needs.

Minimum Qualifications

• Degree from an accredited Physical Therapy program.
• Current Colorado Physical Therapist license.
• New graduates are welcome to apply.
• Experienced Physical Therapists are welcome to apply.
• Ability to provide hands-on patient assessment, treatment, assistance, and exercise instruction.
• Strong communication skills with patients, families, physicians, staff, and leadership.
• Ability to work independently while collaborating with a rehabilitation team.
• Comfortable working across multiple care settings.

Compensation and Benefits
