The Memory Care Center at Well-Spring Solutions is seeking a compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team. This is a unique opportunity to provide nursing care and support to older adults living with Alzheimer's disease, dementia, and other memory-related conditions in a warm, engaging adult day setting.

Unlike many nursing positions, this role offers a predictable schedule with no nights, weekends, on-call responsibilities, or holidays.

Position Summary

As the Health Coordinator, you will serve as the clinical leader within the program and help ensure participants receive high-quality, person-centered care.
Essential Responsibilities

• Complete pre-admission health assessments
• Develop, implement, and update individualized care plans
• Monitor participant health status and respond to changes in condition
• Communicate with physicians, specialists, caregivers, and family members
• Supervise medication administration, first aid, and emergency care
• Coordinate referrals and community healthcare services
• Maintain accurate nursing documentation and health records
• Provide health education to participants, caregivers, and staff
• Support state certification and regulatory requirements
• Assist with staff training, emergency preparedness, and program operations

Qualifications

• Current North Carolina RN or LPN license
• Current CPR and First Aid certification
• Minimum of two years of nursing experience preferred
• Experience in memory care, dementia care, geriatrics, adult day services, home care, assisted living, long-term care, or community health preferred
• Strong assessment, communication, and organizational skills
• Passion for serving older adults and supporting family caregivers
