Key Responsibilities\n

As a Licensed Professional Counselor, your primary responsibilities will include:

• Conducting comprehensive assessments to identify the unique needs of each patient.
\n
• Facilitating clear and timely communication with supervisors, directly involved caregivers, family members, and other stakeholders to discuss patient progress and therapeutic interventions.
\n
• Providing a structured tentative schedule of patient appointments, home visits, and observations to ensure consistent monitoring of treatment progress.
\n
• Coordinating in-home visits to perform observations, conduct chart reviews, engage in staff interviews, and analyze data relevant to patient care and program outcomes.
\n
• Implementing and evaluating program plans to ensure high-quality services are delivered effectively and efficiently.
\n
• Actively participating in Interdisciplinary Team Meetings, contributing insights and expertise to enhance patient care.
\n
• Completing thorough and timely monthly and quarterly progress notes to document patient progress.
\n
• Organizing and facilitating training sessions for staff to ensure competence in program implementation and documentation practices.
\n
• Providing follow-up interviews and ongoing support through home visits and observations to ensure clients’ evolving needs are met.
\n
• Maintaining comprehensive records that ensure transparency and availability of information to all relevant parties.
\n
• Acting as a liaison with the community and mental health networks to foster relationships that support developmental goals.
\n
• Assisting in developing training materials that contribute to the continuous improvement of mental health programs.

Patient Population / Client Focus\n

This role focuses on diverse client demographics, ranging from children and adolescents to adults and families. You will address various mental health challenges, including counseling for trauma, substance abuse, and relationship issues. Your expertise will directly affect the quality of life for individuals and families as you guide them through their recovery journeys.

Qualifications\n

The ideal candidate will have:

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field.

• An active and unrestricted license as a Professional Counselor (LPC) in the state of Georgia.

• A minimum of two years of counseling experience, ideally within a clinical environment.

• A strong understanding of legal and ethical standards governing mental health practice.
\n
• Excellent interpersonal and communications skills for establishing rapport with a varied clientele.

• Demonstrated ability to work cooperatively within a multidisciplinary healthcare team.

• Proficient use of Electronic Medical Record (EMR) systems and related technologies.

Preferred Experience\n

While the basic qualifications are essential, candidates with specialization in family therapy, couples therapy, treatment of substance misuse, or trauma will be highly favored. These specializations will enhance your ability to serve a wide range of clients with complex needs.
