Position Purpose

As a member of the therapy team, the Speech Pathologist's primary function is to provide a thorough speech evaluation, diagnosis, a detailed plan of care including treatment goals and objectives, and consultation with referral sources, client/patient, family members, and other service providers.

Requirements

Qualifications

• Licensed or eligible for licensure by the State of Illinois under the Illinois Speech Pathology Practice Act.
• Member or eligible for membership in the ASHA preferred.
• General knowledge and skill enabling the evaluation and treatment of a wide variety of clients/patients such as neonates, pediatrics, young adults, adults or geriatrics.
• Current certification in CPR and other service line-specific competencies.

Essential Job Functions

• Perform client-centered evaluations and establish a Speech Therapy diagnosis and treatment plan.
• Provide evidenced-based Speech Language Pathology treatment.
• Documentation, reporting, and information dissemination.
• Case management functions: evaluate effectiveness, revise the plan, and discharge from services.
