Registered Nurse (RN) - Home Health Case Manager - Southwest Suburbs

Job Summary Department: 34100 South Metro E - Nursing Location: Oak Lawn, Burbank, Justice, Bridgeview, Hickory Hills, Willow Springs, Chicago Ridge Hours Per Week: 40 Schedule: 7‑day week with weekend and holiday rotation; on‑call rotation; must be able to work one summer and one winter holiday. Pay Range: $38.20–$57.30 per hour ($74,672–$112,112 annual base salary) Benefits: Eligible for comprehensive benefits package, sign‑on bonus up to $10,000 (external candidates only), incentive pay, referral bonus, student loan support, tuition assistance, mileage reimbursement. Responsibilities Provide home care nursing services to the homebound population. Assume primary responsibility for assessment, planning, implementation, and evaluation of nursing care for assigned patients in their homes. Set goals that demonstrate a holistic approach to patients and families. Coordinate all medical needs, including case management and collaboration with social worker, CNA, MD, etc. Document care and connect with the care team via point‑of‑care documentation. Assume leadership responsibilities at the unit level and demonstrate mastery of clinical skills. Develop and maintain a therapeutic relationship with patients and caregivers throughout the care continuum. Facilitate patients’ and families’ right to receive quality care. Qualifications Registered Nurse (RN) license required. Ability to work one summer and one winter holiday, and rotate on an on‑call schedule. Must be employed at the exact position at time of payout to be eligible for sign‑on bonus. Familiarity with home health nursing practices and case management preferred. Compensation & Benefits Annual base salary range: $74,672.00–$112,112.00. Incentive pay eligible. Sign‑on bonus up to $10,000.00 for external candidates, subject to taxes.
