In-Home Geriatric Care RN Case Manager (PACE)
Midland Care Connection, Inc.
Midland Care Connection in Topeka, Kansas is seeking an experienced RN Case Manager to coordinate comprehensive, home-based care for older adults with complex medical needs under the PACE program. You will assess health status, develop individualized plans, and partner with a multidisciplinary team to support independence and quality of life in participants' homes.
