Lead Home Health RN Case Manager
PACE
PACE Southeast Michigan is seeking a Registered Nurse Case Manager to oversee comprehensive nursing care for frail elders in a home-based PACE setting. You will assess needs, plan and implement care with a multidisciplinary team, and coach participants and caregivers to maximize independence. The role requires current MI RN licensure (BSN preferred), strong leadership, and the ability to educate and supervise staff, with on-call duties as needed. #J-18808-Ljbffr PACE
