Hospice RN Case Manager - Lead with Compassion

Gentiva Hospice in Arizona seeks an experienced Registered Nurse Case Manager to lead in-home hospice care, perform assessments, and develop plans of care. You will coordinate with physicians, LPNs, CNAs and social workers to deliver compassionate, patient-centered care, while documenting visits and ensuring regulatory compliance.
