Remote Care Coach: Health Outcomes & Member Support
Humana
Humana Inc. is seeking a Care Coach to evaluate member needs, coordinate care, and drive interventions across Medicare Advantage populations. You will collaborate with members, caregivers, providers, and internal partners to close care gaps and promote treatment adherence.
This remote role requires a 40-hour work week, initial four weeks of training, and a schedule from 10:00 a.m. to 8:00 p.m. ET; travel may be required occasionally.
