Job Summary:

Position Title: Pharmacist Inpatient
Location: Hood River, OR 97031
Duration: 13 weeks
Work Schedule: Day Shift | 4x10s | 7:00 AM - 5:00 PM
Pay rate: 80-85$/hr

Responsibilities for this position include, but are not limited to:

• Verify orders for hazardous and non-hazardous outpatient infusion medications, including chemotherapy.
• Review and verify IV infusion compounding preparations.
• Evaluate oncology and non-oncology treatment plans.
• Perform pharmacotherapy management and therapeutic drug monitoring.
• Verify insurance coverage prior to treatment initiation.
• Provide patient education regarding infusion medications.
• Participate in the management of infusion-related adverse reactions.
• Ensure safe medication dispensing and utilization.
• Collaborate with multidisciplinary healthcare teams across outpatient infusion, oncology, Med-Surg, Labor & Delivery, and Emergency

• IV infusion compounding
• Hazardous drug handling
• Medication order verification
• Therapeutic medication monitoring
• Pharmacotherapy management
• Experience with Epic EMR and Pyxis is preferred.
• Chemotherapy/Hazardous Drug certification is preferred.
• Active Oregon Pharmacist License (Required)
• Minimum 1 year of outpatient infusion pharmacy experience
• Oncology patient care experience is required.
