grade AND Demonstrated Knowledge, Skills, and Abilities Candidates must demonstrate all of the KSAs below in their resume: i Ability to collaborate [and] communicate [with a wide range of medical clinicians across multiple disciplines (e.g., medical doctors, nurse practitioners, physician assistants, psychologists, psychiatrists, social workers, clinical pharmacists, and nursing staff) to accomplish team goal setting to ensure medical care to patients is met.] ii [Ability to independently] set priorities and organize work to meet deadlines, ensuring compliance with established processes, policies, and regulations iii Ability to communicate tactfully and effectively, [electronically, by phone, in person], and in writing, [with internal and external customers] This may include preparing reports in various formats and presenting data to various organizational levels, [as well as resolving patient concerns] iv Advanced knowledge of the technical health care process [(including, but not limited to, scheduling across interdisciplinary coordinated care delivery and/or care in the community models and patient health care portals)] as it relates to access to care v Advanced knowledge of policies and procedures associated with interdisciplinary coordinated care delivery and/or care in the community operational activities that affect patient flow, and patient support care [administrative functions] to include, but not limited to appointment cycles, outside patient referrals, follow-up care, overbooking, provider availability, etc vi Advanced knowledge of vacancy is GS-06 Preferred Experience In addition to the minimum qualifications, the following criteria are preferred: Experience working within a Patient Aligned Care Team (PACT) or interdisciplinary care team model, including participation in team huddles and team meetings to plan and manage patient care in coordination with providers, nurse care managers, and clinical associates Demonstrated experience coordinating patient care communications across multiple points of contact, such as facilitating secure messaging, communicating with non-VA medical facilities, and managing inpatient-to-outpatient discharge coordination Experience developing and maintaining tracking systems for follow-up care (e.g., consults, diagnostic tests, pre-appointment requirements) to ensure continuity of care and patient readiness for visits or procedures Experience using electronic health record (EHR) systems to manage clinic operations, including monitoring wait lists, validating scheduling accuracy, and reviewing clinic utilization reports to support team and patient needs Physical Requirements: Physical aspects associated with work required of this assignment are typical for protective equipment and undergo annual TB screening or testing as conditions of employment Work Environment: Work is performed in an office/clinic setting with minimal risks that requires normal safety precautions the area is adequately lighted, heated, and ventilated However, the work environment requires someone with the ability to handle several tasks at once in sometimes stressful
