RN Case Manager/Preceptor - Home Health

Connected Home Health is seeking experienced & caring Home Health Registered Nurse (RN) Case Managers with precepting experience to join our awesome team! Do you desire a flexible work schedule? Connected serves patients in the greater Portland, OR area with territories available on the Southeast, Southwest, Northeast and Northwest. The ideal candidate is a dedicated patient advocate who is home health savvy, compassionate, experienced working with an interdisciplinary team, flexible, driven, creative, technologically adept, fun (!), quality focused, positive, and overall awesome. Our Registered Nurses are supported by trained LPNs.Salary: commensurate with experience $53 +Essential Job Functions/ResponsibilitiesProvides direct patient care as defined in Oregon’s Nurse Practice ActImplements plan of careProvides accurate and timely documentation consistent with the plan of careAssesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of careParticipates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nursePosition QualificationsGraduate of an accredited practical nurse or vocational nursing programOne year nursing experience preferredCurrently licensed as a licensed registered nurse in the stateComplies with accepted professional standards and practiceDemonstrates good verbal and written communication, and organization skillsMust be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working orderPossesses and maintains current CPR/BLS certification from AHAConnected Home Health is an Equal Opportunity Employer.
