Case Management Liaison

Job Title Case Management Liaison Reporting To Assistant Nurse Manager – Care Coordination Department Case Management Pay Days Pay range: $23.37 – $32.71. Relief Differential: 15%. Position Overview The Case Management Liaison works directly with unit Nurse Case Manager(s) and Social Service Specialist(s) and reports to the Manager of Nurse Case Management & Nurse Navigation. The liaison supports the case management team in completing required forms/documentation, coordinating care and arranging post‑acute follow‑up, ensuring patients receive exceptional care and avoid delays in discharge. This position does not directly manage other caregivers. Essential Functions & Duties Support discharge planning activities under the direction of the Nurse Case Manager or Social Service Specialist. Comply with all documentation requirements; thoroughly document all work completed within the medical record. Identify primary care provider or specialist for follow‑up appointments and schedule them as appropriate. Provide choice lists to patient or caregiver for post‑acute services and information on community resources, indigent programs, and temporary housing. Communicate with vendors, physician offices and clinics for discharge planning purposes. Submit referrals for post‑acute services and confirm payor authorizations for placements, home health, home care, DME, etc. Follow‑up with post‑acute agencies to identify patient acceptance and finalize communication of services with patient or caregiver. Arrange post‑discharge transportation and coordinate medication delivery from onsite pharmacy as needed. Present and explain regulatory notices (Medicare Letter, Beneficiary Notification Letter) and obtain patient or caregiver acknowledgment. Identify any post‑acute delay/avoidable days tied to placement and report or escalated to Nurse Case Manager or Social Service Specialist. Provide information to payors related to discharge planning activities. Fax, copy or scan EMR information for discharge planning purposes. Support the organization’s vision, mission and values, and Value Improvement Practice (VIP- Lean) principles. Maintain a safe environment for caregivers, patients and guests, and conduct all activities with professionalism and confidentiality. Comply with all applicable laws, regulations, policies and procedures, report violations, and cooperate with investigations. Deliver customer service and/or patient care that promotes goodwill, timeliness and accuracy. Education Required: High school diploma or equivalent. Preferred: Associate’s degree or higher. Licensure / Certification / Registration No specific requirement. Experience Required: One year of relevant experience in healthcare or service environment. Preferred: Prior case management experience. Physical Requirements Daily activities include clear speaking voice, normal hearing, sitting, standing, walking, lifting up to 10 pounds, keyboard operation, bending, reaching overhead, carrying/pushing/pulling 1‑10 pounds; occasional tasks of 11‑15 pounds; rare vehicle operation; never lifting 25‑50 pounds. No exposure to heat, cold, wet areas, noise, vibration, or chemicals. No risk for blood‑borne pathogen exposure.
