Office Location: 131 Temple Lake Drive, Suite 2 Colonial Heights, VA 23834
Coverage area: Travel required throughout the Richmond, VA location territories
Schedule: Full Time Monday-Friday 8AM-5PM

As a RN Case Manager You will:
- Assess/monitor physical, emotional, and psychological needs of patients
- Create hospice care plans that align with the patients wishes and goals
- Direct nursing care: administering medications, treatments, and interventions
- Provide pain and symptom management
- Educate and support the patient’s family and caregivers
- Collaborate with interdisciplinary team
- Maintain accurate and timely
