About the Role:
Care Essentials Home Health is looking for a compassionate and organized Patient Service Coordinator to join our dedicated team in San Bernardino, AL. In this vital role, you'll be the connective thread between patients, caregivers, and clinical staff — ensuring every patient receives seamless, high-quality home health care. Responsibilities:
- Coordinate and schedule home health services for patients, ensuring timely and accurate care delivery
- Serve as the primary point of contact for patients, families, and referral sources
- Manage patient intake process, including verifying insurance eligibility and authorizations
- Maintain accurate patient records and documentation in compliance with healthcare regulations
- Communicate effectively with clinical staff, physicians, and care teams to facilitate patient care plans
- Monitor patient service schedules and resolve scheduling conflicts or gaps in care
- Support billing and coding processes by ensuring proper documentation and authorizations are in place
Requirements:
- 1+ years of experience in a healthcare, home health, or patient coordination role preferred
- Familiarity with home health regulations, insurance verification, and prior authorizations
- Strong organizational and multitasking skills in a fast-paced environment
- Excellent verbal and written communication skills with a patient-first attitude
- Proficiency with electronic health record (EHR) systems and scheduling software
- Compassionate, detail-oriented, and able to problem-solve under pressure
- High school diploma or GED required; associate's or bachelor's degree in healthcare administration a plus
