Job Summary:
The wound care nurse practitioner will be responsible for conducting health assessments focused on wound, ostomy, and continence-related care in a healthcare organization in New York. This role involves developing and evaluating care plans based on assessments, collaborating with the healthcare team, and providing patient and staff education to promote optimal outcomes. The position requires applying clinical expertise in wound and colostomy care within a fast-paced medical-surgical environment.

Job Details:

• Schedule: Four shifts per week (10 hours each) with every other weekend.
• Required Qualifications: Associates Degree (ASN, ADN) from an accredited Nursing Program; New York State Registered Nurse (RN) license with primary source verification; AHA Basic Life Support (BLS) certification; two years of Med/Surg experience; wound and colostomy care experience.
• Preferred Qualifications: N/A
• Additional Information: Responsibilities include assessing and documenting patient responses to wound, ostomy, and continence treatments, evaluating clinical staff performance, and developing staff education programs. The role requires collaborative efforts with the nursing leadership to implement best practices and participate in quality improvement activities. Culturally competent patient, family, and staff education, as well as asset management related to rented equipment, are key operational aspects of this position.
