Field Care Manager, Behavioral Health

Humana is hiring a Field Care Manager, Behavioral Health to support members with complex mental health needs in the community. In this role, you’ll complete biopsychosocial assessments, develop person-centered care plans, coordinate services across medical, behavioral, and social supports, and monitor progress toward goals. You will collaborate closely with providers, families, and community resources, conduct field and virtual visits, and provide crisis intervention and education. You’ll document in electronic systems, use data to guide interventions, and champion Humana’s member-focused, inclusive culture.
