Assess changes in the patient’s condition and promptly communicates significant findings to the RN Case Manager, physician, and/or appropriate members of the interdisciplinary group (IDG). Provide nursing care in accordance with the established plan of care and assists with implementing updates or interventions based on changes in the patient’s condition. Administer medications and treatments as prescribed by the physician and in accordance with hospice policies and nursing standards of practice. Provide interventions to support effective pain and symptom management and evaluates the patient’s response to treatment. Educate patients, families, and caregivers regarding medications, symptom management, comfort measures, disease progression, and when to contact the hospice team for additional support. Coordinate with the RN Case Manager and other IDG members to ensure continuity of care and appropriate follow-up after PRN visits. Respond to changes in patient condition, symptom‑management needs, and other urgent clinical concerns within the scope of the PRN assignment. Complete timely and accurate documentation of assessments, interventions, patient/family education, changes in condition, and communication with the hospice care team. Provide compassionate, patient-centered end-of-life care that promotes comfort, dignity, and support for patients and their families.
