Description

• Conduct thorough evaluations for patients in both inpatient and outpatient settings, crafting personalized treatment plans
• Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Collaborate with an interdisciplinary team including nurses, physicians, case managers, and rehabilitation specialists to achieve optimal patient outcomes
• Educate patients and their families on therapeutic objectives, discharge protocols, and at-home exercise regimens
• Maintain comprehensive documentation of evaluations, treatments, and progress in line with regulatory standards and facility policies
• Engage in collaborative care team meetings and interdisciplinary conferences to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification strongly preferred
• Additional specialty certifications (e.g., in geriatrics or neurology) are advantageous but not essential

Skills

• Previous experience in a clinical setting preferred
