Work Shift: 8 Hours - Day Shifts (United States) Scheduled Weekly Hours: 32 Compensation: Salary Range: $25.50 - $36.00 Union Position: No Department Details Summary: Provides supportive services and counseling on healthcare and home care programs and services. Serves as a member of the interdisciplinary team in providing assistance with social, emotional, and economic concerns of patients, clients, residents, families, and caregivers, thereby enabling them to achieve or maintain an optimal level of functioning through coordination and planning of programs. Key Responsibilities Provide crisis intervention and assist families in understanding the implications and complexities of medical situations. Coordinate healthcare programs among patients, clients, residents, families, caregivers, and the psychosocial and healthcare teams and communities. Apply knowledge of human behavior and developmental stages in client interactions. Respond to suspected abuse, neglect, or violence in accordance with the National Association of Social Work Code of Ethics and state laws. Develop an appropriate plan of care for patients, clients, residents, and families by obtaining resources from social, health, and human services agencies. Provide referrals, current information, and/or education regarding available programs and services. Demonstrate commitment to the organization by utilizing time effectively, participating in special projects and assignments, and exhibiting flexibility when necessary. Show professionalism through participation in care conferences and transitional rounding, serving as an advocate. Apply critical thinking, problem solving, and decision‑making skills effectively. Maintain clear written and verbal communication skills while establishing rapport with patients, clients, residents, families, caregivers, communities, and healthcare teams. Display independent judgment and actively participate with the healthcare teams. Depending on the department, provide social services for donors and transplant recipients. Qualifications Bachelor’s degree in Social Work or a related field from an accredited curriculum (required). Healthcare and/or mental health hospital experience preferred. Basic Life Support (BLS) certification required within six weeks of employment (depending on location). Multi‑state licensure privilege may be required depending on position. Must possess a license in good standing in state(s) of practice. For Wisconsin: Wisconsin Social Work Certification or Social Work Training Certificate within two years of hire date, leading to obtaining the state license.
