Oasis Therapy Services is seeking a Physical Therapist to provide one-on-one rehabilitation services in Apple Valley, Minnesota.

Available employment option:

• Part-Time

EMR: Net Health

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide treatment focused on strength, balance, gait, transfers, mobility, pain management, and fall prevention.
• Monitor residents' progress and modify treatment plans as needed.
• Complete discharge planning and provide recommendations for continued care.
• Educate residents, families, and caregivers on exercises, safety, and assistive equipment.
• Supervise Physical Therapist Assistants when assigned.
• Complete accurate Net Health documentation and collaborate with the care team.

Qualifications

• Active and current Minnesota Physical Therapist license.
• Graduate of a CAPTE-accredited Physical Therapy program or a program that meets CAPTE requirements.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, senior living, or rehabilitation experience is preferred.
