Housing Stability Case Manager
Avenue 360 Health and Wellness
Avenue360 Health and Wellness seeks a dedicated Housing Case Manager to support permanent supportive housing participants with comprehensive case management and services. You will coordinate with internal teams, property owners, and community partners to promote housing stability and prevent homelessness. Role requires a bachelor’s degree or commensurate experience, 2 years of case management or housing experience, and strong interpersonal and communication skills. #J-18808-Ljbffr
