Responsibilities
- Perform initial PT evaluations and develop individualized treatment plans for residents in the skilled nursing setting.
- Deliver evidence-based physical therapy interventions to improve mobility, strength, balance, and functional independence.
- Monitor patient progress, document outcomes, and adjust treatment plans in collaboration with the interdisciplinary team.
- Educate patients and families on home programs, safety, and fall-prevention strategies; ensure patient safety during therapy sessions.
- Maintain accurate, timely documentation in the medical record and adhere to all facility policies and regulatory requirements.

Requirements
- Active physical therapy license in good standing; NH licensure or eligibility may be required per regulatory standards.
- Doctor of Physical Therapy (DPT) degree or equivalent recognized by state licensing authorities.
- Basic life support (BLS) certification or per facility policy.
- Experience in skilled nursing facility or geriatric rehab settings preferred; strong communication and documentation skills.
- Ability to work effectively within a multidisciplinary team and adapt to a fast-paced care environment.
- Willingness to travel and commit to a 13-week assignment; able to meet credentialing and onboarding requirements.
